As we age, declining circulation and hormonal levels along with cumulative ultraviolet exposure causes our bodies to produce less essential components in the skin to keep it soft, supple and youthful. This includes a decline in the production of collagen, elastin and hyaluronic acid. Fat compartments beneath the skin also change position and lose volume. Deeper yet, the facial skeleton which is the firm foundation for the facial anatomy changes in shape as bone undergoes resorption. Thinning, loss of ground substance and elasticity of the skin leads to the formation of fine lines and wrinkles. Loss of bone and changes in fat compartments leads to loss of projection. - Soften lines of the forehead often used in conjunction with botox and after other texture treatments like laser resurfacing.

WHAT AREAS OF THE FACE CAN BE ENHANCED?

- Dermal fillers for the cheeks
Several dermal fillers like Juvederm, Voluma, Radiesse and Sculptra are injected in the midface by Dr. Fink to restore volume and enhance the cheek area. Multiple injections are performed with very small needles to restore definition and fullness. Filling and elevating sagging skin soften folds and creases around the mouth and lifts the jowls.
- Dermal Fillers of the lips
Dermal fillers into the lips are the safest and most reliable procedure to enhance the shape and add volume to the lips. Lips anatomy consists of the vermilion or red part, the white roll or border of the vermilion, the two vertcal ridges beneath the nostrils (philtral columns), and the dip between the philtrum at the mid upper lip border (cupids bow) and all can be enhanced with fillers. Dr. Stephen Fink evaluates each point of the lip anatomy and injects fillers in the areas that give you naturally enhanced lips that are not overdone.
To increase the volume of the lip, Dr. Fink injects the body of the lip vermilion. This everts the lip outward to create more pout and projection of the red portion. Generally speaking, naturally appearing lips maintain a 2/3 to 1/3 ratio of the lower lip to the upper lip.
To enhance and define the contour of the lip border, Dr. Fink injects filler into the junction between the white and red part of the lips. This also helps the vertical wrinkles or so called “lipstick bleed lines”.
For the flattened upper lip, Dr Fink enhances the two vertical philtral columns. This restores the lips shape and curve creating a more youthful and sensual look. The cupids bow or dip between the columns at the lip border can also be enhanced to achieve a pleasant appearance to the lips.
To improve the down turned corners of the mouth ( the Oral Commisures), Dr. Stephen Fink injects fillers into this area to support and lift the corners upward creating a happy appearance. In some cases, Botox is combined with fillers to relax the muscles that pull the corners of the mouth downward.
- Dermal Fillers for lines of the Forehead and Between the Eyebrows
For deep vertical lines of the forehead and the vertical “11′s” between the eyes, fillers like Belotero and Restylane can soften the residual lines that are unresponsive to Botox and other treatments for skin texture. Droopy eyebrows and excess skin of the upper lids can also be improved by adding volume to the forehead.

Usually second to the facial appearance, your hands can also reveal your age. Volume loss results in prominence of the tendons and veins on the back of the hands. Sun exposure further thins the skin. IPL treatments can also be added to lighten the pigment and discoloration.

Certain types of depressed scars secondary to injury and acne can be improved with dermal fillers. By injecting a small amount of filler under the depressed area of the skin, Dr. Fink can improve the appearance. Microneedling with PRP and other radiofrequency needle treatments can also improve these conditions.
WHAT HAPPENS DURING TREATMENT?
Dr. Stephen Fink will discuss your treatment plan to make you comfortable and at ease with the procedure. Dr. Fink will then mark some anatomical landmarks on the skin to help with accurate placement of the injections. Topical or local anesthesia may be utilized and some fillers may be premixed with lidocaine. Then an ultrafine needle or a smooth tipped canula is used to inject dermal fillers into skin lines, creases and underneath the skin with minimal discomfort. The area may be iced before and after the injections to help minimize bruising. At the end, Dr, Fink may massage the treatment area to smooth out bumps or irregularities.
HOW DO DERMAL FILLERS WORK?
Dermal fillers work by adding volume in the area where they are injected. When injected at skin lines or creases, they lift and support under the creases to unfold them and smooth the surface.When injected deep on top of bone, they lift up the soft tissues (muscle,fat and skin), augmenting the contour like a facial implant. Certain fillers contain a substance called Hyaluronic acid which is native to the skin. This substance attracts 100 times it’s volume in water which adds hydration to the area. Some fillers may also contain substances like calcium or other inert molecules that cause a mild reaction leading to the production of new collagen over the course of time.
HOW LONG WILL MY RESULTS LAST?
The duration of dermal fillers varies according to the type of filler and the patient. The amount of movement in an area of injection can also affect the longevity. Most fillers last between 8 months and up to 2 years.
WHAT ARE THE MOST COMMON SIDE EFFECTS OF DERMAL FILLERS?
mmediately following the injections, you may experience some mild redness, swelling or bruising in the treatment area. These symptoms typically disappear within a few days. Allergic reactions are extremely rare. Infrequently, firm areas known as granulomas can form and should be evaluated by your doctor. Most people are able to resume normal activities immediately following the treatment.
WHAT SHOULD I DO BEFORE TREATMENT WITH DERMAL FILLERS?
To lessen bruising, it is best to refrain from using medications that thin the blood at least one week before treatment. Medications and over the counter supplement include products that contain aspirin, arthritis or antiflammatory medications like Ibuprofen (motrin and advil), Aleve and Naprosyn etc., Fish Oil and Omega -3s, Vitamin E and many other supplements. Prescription blood thinners should only be stopped with the permission of the prescribing physician. Acetaminophen (Tylenol) can be safely taken prior to treatment since it doesn’t increase the risk of post injection bruising.
WHAT SHOULD I DO AFTER TREATMENT WITH DERMAL FILLERS?
Ice to the treatment area is recommended for 6-10 hours afterward. Exercise should be avoided for at least 24 hour after injections. Exercise temporarily raises blood pressure that increases the risk of bruising. Medications and supplements that thin the blood should continue to be avoided for 48 hours after treatment. Avoid further massage or manipulation of the treatment areas.
